2011-2012 REGISTRATION

Registration for returning students who completed the 2011-2012 school year

All students who completed the 2011-2012 school year should have received a re-registration package prior to the year end shows. If you have misplaced your personalized package, please print the general documents below and return them to the school as soon as possible.

Registration for new students

MHSB is still accepting new registrations for our fall programs. Please be aware that some classes may have already been filled. Students over the age of 12 and older may need to be assessed for an appropriate level.

Registration documents (in PDF format) for in person or mail in registration:

In order to register, MHSB requires:

  • A completed registration form. Please review the School and Studio and Withdrawal Policy documents - you must indicate that you have reviewed and agree to the terms in these documents when signing your registration form.
  • A completed Class request and Credit Card Authorization form

Ways you can register:

  • In person at our 2384 Yonge St. location, click here for office hours

  • By scanning and emailing your regstration form to office@mhsb.ca
  • By faxing your forms to (416) 486-3193. Please follow up your fax with a call or email to ensure it was received
